It's a short tutotial for You:
1. Prepar the data in excel or where else
The first line is the name of the variables through ":" its type and the number of allocated symbols, the field delimiter is TAB (tab), for example:
Zip: varchar: 5 city: varchar: 30

Where zip is the variable name that can be used in posts {zip}. VARCHAR is a string value, and 5 is the maximum number of characters ( 5 characters maximum)

After the first line with the headers goes data, also separated by tabulation (TAB key)

2. Save the data to a file with the extension txt or csv, under any name
3. Go to the plugin in the section Global variable and click on the link "Create Set"
4. Enter the name of the set, it will be displayed in the posts where you select country (set) to use for generation
5. Select the file and click "download"
